Our Children’s Behavioral Health Home providers deliver wraparound, person-centered care coordination and support to children.
This comprehensive care team includes Care Coordinators, Family Support Navigators, and Registered Nurses who work together to assist members in accessing and coordinating medically necessary services both in the home and throughout the community.
Our team collaborates closely with children, families, providers, schools, and community resources to develop, implement, and monitor individualized plans of care tailored to each child’s unique strengths and needs.
These services are designed to promote health, safety, independence, and community inclusion while providing education, advocacy, resources, and ongoing support to children and their families.
