This comprehensive care team includes Care Coordinators, Care Managers, and Clinical Nurses who work together to help members access and coordinate medically necessary services both at home and throughout the community.
Our team collaborates closely with adults, their support networks, providers, employers, and community resources to develop, implement, and monitor individualized plans of care tailored to each person’s unique strengths and needs.
These services are designed to promote health, safety, independence, and community integration while providing education, advocacy, resources, and ongoing support to adults and their support systems.
